DNA Vaccine that treats immunosuppressed

We are testing a vaccine, which consists of DNA that directs human cells to produce a protein that stimulates the body’s immune system to generate protective antibodies against the Human Immunodeficiency Virus (HIV).
In this study we wish to work out the best conditions for using this DNA. Previous animal and human studies have shown that antibody responses to DNA vaccines can be influenced by the dose (amount of DNA), route of injection (into the muscle or skin), and use of electroporation (EP), which consists of mild millisecond electrical pulses at the site of injection.
This greatly increases the amount of vaccine getting into cells, playing an important role in how well a vaccine works. Combining this vaccine with another type of DNA that promotes production of a natural human protein (recombinant IL-12) may further improve the way the vaccine works.
Similar DNA vaccines have been tested in a number of clinical trials and found to be safe.
In this study the vaccine will be administered by injection into the muscle (intramuscularly), skin (intradermally) or both (in thigh and/or upper arm) with and without DNA for recombinant interleukin 12, together with electroporation.
We will invite healthy men and women to take part in this research. A total of 40-48 will be enrolled and each will receive four vaccinations over the course of 4-6 months. We will assess the effects of the vaccinations by recording any symptoms and by analysing blood samples over 6-9 months depending on vaccination schedule.
The vaccine is already being studied in a small numbers of men and women by the IM route and to date has not caused any serious adverse events. However it has not previously been administered by electroporation with or without DNA for IL-12. Safety and efficacy will therefore be monitored extremely closely.
Of course, no one could foresee any of all this MRNA Vaccine technology malarky...

Professor Robin Shattock, Head of Mucosal Infection and Immunity within the Department of Medicine, is working on the manufacturing of RNA vaccines to create quicker and more accessible responsiveness to outbreaks of known pathogens - such as flu, and unknown pathogens, called Disease X.
His team is improving the production system of vaccines to quickly provide tens of thousands of new vaccine doses within weeks of a new threat being identified. Currently, vaccines can take 10 years or more to develop.

Synthetic RNA vaccines – which harness the body’s own cell machinery to make an antigen rather than injecting the antigen directly – take only weeks to produce and may offer the easiest route for global product consistency.
This approach is already being successfully used in cancer. This technology provides the ability to manufacture anywhere in the world and Professor Shattock is prototyping it in the Future Vaccine Manufacturing Research Hub.
Professor Shattock said: “In a pandemic situation, for every month delay, there is a potential for five million fatalities, that’s equivalent of losing a city the size of Rome or Singapore every month. We need to completely reimagine the way vaccines are being produced and approved.”

Professor Wendy Barclay, Medical Research Chair in Virology at the Department of Medicine, is aiming to understand whether there is a potential to develop disease control through new measures of animal protection, exploring transgenic animals and how they could impact vaccine spread.
The source of all influenza is wild water birds, like ducks and geese. The avian virus can cross into farmed animals, chickens and pigs, which are a stepping stone for the virus to jump into humans.
People exposed to infected chickens can catch ‘bird flu’, which is often fatal.
Professor Barclay believes that by preventing influenza virus crossing from wild birds into chickens, we would stop the next pandemic at source.
Working with the Roslin Institute, Professor Barclay is using CRISPR technology - a way to precisely alter the genome of a species to introduce beneficial new traits - to develop chickens that are totally resistant to infection by influenza virus.
Professor Barclay said: “With our idea to generate farmed animals that cannot be infected by influenza viruses we aim to bring global health security by stopping influenza pandemics from emerging.”
Heat-stable vaccines for use in remote and low-income settings
Professor Jason Hallett, Professor of Sustainable Chemical technology at the Department of Chemical Engineering, is working on thermal stabilisation of vaccines allowing them to exist in room temperature without destroying their efficacy.
Replacing the standard vaccines with heat-stable versions would lower costs and enable them to be used in challenging conditions, such as in regions where the cold-chain that current vaccines require, cannot be maintained.
Up to 80% of the cost of vaccinating a person is due to cold storage at negative 50 degrees. This temperature has to be maintained when shipping vaccines across the world. The cold chain is needed to stop vaccines from being destroyed by heat and water. This gives rise to a shelf life at room temperature – a maximum amount of time vaccines can be safely stored.
Professor Hallett has designed salts that could act like water and protect the vaccine structure even when hot.
Professor Hallett said: “If we could eliminate the need for cold storage, then we could produce vaccines anywhere in the world. We could ship them and store them indefinitely. This really matters when you don’t live in a major city.”

This summer, computational biologist Luke Hutchison volunteered for a trial of Moderna's COVID-19 vaccine. But after the second injection, his arm swelled up to the size of a “goose egg,” Hutchison says. He can't be sure he got the vaccine and not a placebo, but within a few hours, Hutchison, who was healthy and 43, was beset by bone and muscle aches and a 38.9°C fever. “I started shaking. I had cold and hot rushes,” he says. “I was sitting by the phone all night long thinking: ‘Should I call 911?’”
Hutchison's symptoms resolved after 12 hours. But, he says, “Nobody prepared me for the severity of this.”
He says the public should be better prepared than he was, because a subset of people may face intense, if transient, side effects, called reactogenicity, from Moderna's vaccine. Some health experts agree.
“Somebody needs to address the elephant: What about vaccine reactogenicity? While it's … not going to cause any long-term issues … how is that perception going to go with the public once they start receiving it?” asks Deborah Fuller, a vaccinologist at the University of Washington, Seattle, whose lab is developing second-generation RNA vaccines against COVID-19. She worries the side effects could feed vaccine hesitancy. “I feel like it's being glossed over.”
Those concerns arise after a week of good news about coronavirus vaccines: Both Moderna and Pfizer, with BioNTech, announced their messenger RNA (mRNA) vaccines reached 95% efficacy in clinical trials of tens of thousands of people. The firms added that the trials showed no serious safety concerns.
Both vaccines consist of a snippet of genetic code directing production of the coronavirus' spike protein, delivered in a tiny fat bubble called a lipid nanoparticle. Some researchers suspect the immune system's response to that delivery vehicle is causing the short-term side effects.
Those transient reactions should not dissuade people from getting vaccinated in the face of a pandemic virus that kills at least one in 200 of those it infects, says Florian Krammer, a vaccinologist at the Icahn School of Medicine at Mount Sinai, who participated in Pfizer's trial. Sore arms, fevers, and fatigue are “unpleasant but not dangerous,” he says. I'm not concerned about [reactogenicity] at all.”

COVID-19 immunity 'could last three months after infection or vaccination'
The government's scientific advisers say the low numbers being reinfected suggests "natural immunity is sustained in most cases".

People who receive a COVID-19 vaccination may have immunity for 90 days, the government’s scientific advisers have said.
In a paper considered by the Scientific Advisory Group for Emergencies (SAGE), which also looked into the much-discussed concept of immunity certificates, the experts say that the length of immunity from "natural infection or vaccination is currently not known".
However, they say that "waning immunity" is believed to partly cause other coronaviruses to reinfect after one to two years.
Meanwhile, seasonal coronaviruses frequently re-infect people 12 months later and sometimes as early as six months on, the scientists say.

"We conservatively estimate that a protective immune response after SARS-CoV-2 infection or vaccination may last for 90 days (moderate confidence)," they say.
"Based on this information, we conclude that: Within one month of natural infection, a high proportion of people will develop immunity which is protective against disease caused by reinfection (high confidence).

"The old adage 'you are what you eat' is a touchstone for many aspects of human health," said Lawrence. "But in terms of the body's ability to fights off infections, this study suggests that, to a certain extent, you may also be what your great-grandmother ate."
While other studies have shown that environmental exposure to pollutants can have effects on the reproductive, respiratory, and nervous system function across multiple generations, the new research shows for the first time that the immune system is impacted as well.
This multigenerational weakening of the immune system could help explain variations that are observed during seasonal and pandemic flu episodes. Annual flu vaccines provide some people more protection than others, and during pandemic flu outbreaks some people get severely ill, while others are able to fight off the infection. While age, virus mutations, and other factors can explain some of this variation, they do not fully account for the diversity of responses to flu infection found in the general population.
"When you are infected or receive a flu vaccine, the immune system ramps up production of specific kinds of white blood cells in response," said Lawrence. "The larger the response, the larger the army of white blood cells, enhancing the ability of the body to successfully fight off an infection. Having a smaller size army -- which we see across multiple generations of mice in this study -- means that you're at risk for not fighting the infection as effectively."

In the study, researchers exposed pregnant mice to environmentally relevant levels of a chemical called dioxin, which, like polychlorinated biphenyls (PCBs), is a common by-product of industrial production and waste incineration, and is also found in some consumer products. These chemicals find their way into the food system where they are eventually consumed by humans. Dioxins and PCBs bio-accumulate as they move up the food chain and are found in greater concentrations in animal-based food products.
The scientists observed the production and function of cytotoxic T cells -- white blood cells that defend the body against foreign pathogens, such as viruses and bacteria, and seek out and destroy cells with mutations that could lead to cancer -- was impaired when the mice were infected with influenza A virus. This weakened immune response was observed not only in the offspring of the mice whose mothers where exposed to dioxin, but in the subsequent generations, including as far out as the rodent equivalent of great-grandchildren. The researchers also found that this effect was more pronounced in female mice.
The study authors' hypothesis that the exposure to dioxin -- which binds a protein in cells called AHR -- in some fashion alters the transcription of genetic instructions. The exposure itself does not trigger a genetic mutation, rather the cellular machinery by which genes are expressed is altered and this phenomenon is passed onto subsequent generations.

An Israeli-American pharmaceutical company is preparing to launch a Phase I clinical trial for what could become the world's first oral COVID-19 vaccine.
Oramed Pharmaceuticals Inc., a clinical-stage pharmaceutical company based on technology developed by Hadassah-University Medical Center, announced over the weekend a joint venture with India-based Premas Biotech to develop a novel oral vaccine. Together they formed the company Oravax Medical Inc. The vaccine is based on Orameds POD oral delivery technology and Premas's vaccine technology.

Some hope that the technology will help patients and doctors to track their
drug regimen compliance, increasing patient adherence. Estimations suggest
this could save around $100 – $300 billion annually in the US, whilst other
have concerns about the privacy of patients, consent and data sharing.
An expert in legal and ethical issues surrounding health care technology,
Associate Professor Eric Swirsky suggested that both groups have valid
arguments, but that neither group is asking the right questions.
Prof Swirsky is
the clinical associate professor of biomedical and health information sciences
in the UIC College of Applied Health Sciences.
“We need to know if smart pills are going to actually improve patients’ lives,
which is much more complicated than compliance or privacy,” he said. “It is
naive to think that this type of surveilled compliance with provider-
recommended drug treatments will function like a magic pill. More likely, it will
just challenge the ingenuity of patients.”
He claimed that there was no concrete evidence that suggested smart pills
benefit patients, or that the use of the technology outside of clinical trials is
beneficial.
“Smart pills are a dangerous reduction of the provider-patient relationship and
there is no shortcut to improving patient adherence, which happens in a larger
framework of home, work and clinical environments, not to mention
perceptions and emotions,” he added.
“This technology dumbs down an issue that is often very complex in the hopes
of quickly solving an expensive medical challenge.

Exposed: NHS vaccine passports sold to jab refuseniks for £750 as criminals use Telegram to advertise and upload 'certificates' to buyers' mobiles

Posing as a potential buyer, our reporter contacted Doctor J and asked if he could 'sort an NHS vaccine certificate'.
The fraudster not only agreed, but boasted that he could arrange the manipulation of an individual's NHS records to make it falsely appear as if they had been double jabbed.
'It's not actually just the certificate,' he declared. 'It will be on the system that you've had both jabs with a real date, a real batch number.
'So the way it works you obviously have to download the NHS App, send us your full name, date of birth and NHS number and then obviously we input it on the system and … within 48 to 72 hours it will be up on the app ready to use.'
He claimed to have processed ten vaccine certificates that week, with another five awaiting his attention the next day.
After agreeing to buy the pass, the reporter provided details of an individual who had not been vaccinated for medical reasons and who had agreed to help expose the scandal. Three days later, Doctor J sent a message apparently showing that our buyer's NHS records had been altered and now falsely showed the dates of two Covid vaccinations – one on June 27 and another on August 16.
Within a further 24 hours, our buyer's NHS app had been updated and now included a fraudulent vaccine pass.
Astonishingly, the app falsely stated that our buyer had received two Pfizer-BioNTech Covid vaccination doses, each of which had its own vaccine batch number.
It also wrongly claimed that he had been given the jabs at the St Charles Centre for Health, a large clinic in Notting Hill, West London.
An official vaccine pass QR code had been generated on the app for use both domestically and for international travel, while an official document displaying the pass had also been downloaded.
Smartphones could be used to control peoples ability to access areas & general movements by disabling the app.
